Creatine Beyond Muscle: Brain, Fatigue, and Aging

Creatine built its name in the weight room. The research on what it does for your brain and your years is smaller, more mixed, and more interesting than the marketing lets on.

Creatine is the most studied supplement on the shelf, and for decades the pitch was simple. Take it, lift, get bigger. That part is settled.

Then the conversation moved. Now you hear that creatine sharpens your thinking, fights brain fog, protects you as you age, and works even if you never touch a barbell. Some of that is real. Some of it is a small effect wearing a big headline. Here is the honest split.

Why your brain cares about creatine at all

Start with the mechanism, because it is the part that actually holds up. Creatine is not a stimulant and it is not a hormone. It is part of your body's rapid energy recycling system. It helps regenerate ATP, the molecule your cells spend to do work.

Your brain runs that same system. It is a metabolically expensive organ, and it keeps its own creatine stores. Reviews of this area lay out the logic plainly: if creatine helps recycle energy in muscle, and the brain uses the same machinery, then topping up brain creatine might help in situations where brain energy is under strain (Roschel et al., 2021).

That review is careful about the limits, and so should you be. The optimal protocol for actually raising brain creatine is still not settled, and brain stores are harder to move than muscle stores. The mechanism is real. That does not automatically make the benefit large.

Memory: a small effect, biggest in older men

This is where the evidence is strongest, and it comes with a specific shape.

A meta-analysis pooled 10 randomized controlled trials of creatine versus placebo on memory in healthy people, with 8 of them in the statistical analysis. Overall, creatine improved memory measures, but the effect was small. The interesting part was the split by age. In older adults aged 66 to 76, the benefit was clear and much larger. In younger people aged 11 to 31, it was essentially zero. Dose, duration, and sex did not change the picture (Prokopidis et al., 2023).

A separate systematic review of six trials came to a compatible conclusion. Short-term memory and reasoning may improve with creatine, but results across other domains like attention, executive function, and reaction time were conflicting, and performance in young individuals mostly stayed flat. The authors flagged aging and stressed people as the most likely beneficiaries (Avgerinos et al., 2018).

Read those two together and the honest summary is this. If you are a healthy, well-rested 28-year-old, do not expect creatine to make you smarter. If you are older, or your system is under strain, there is more reason to expect something.

The biggest trial pumped the brakes

Then the largest study on this question landed, and it is worth taking seriously precisely because it was well designed.

Researchers ran a preregistered, crossover, double-blind, placebo-controlled trial in 123 participants, giving 5 grams a day for six weeks. They tested reasoning with Raven's Advanced Progressive Matrices and working memory with the Backward Digit Span, plus eight more exploratory tests. The result was underwhelming. The working memory effect only bordered on significance. The reasoning test showed nothing. None of the eight exploratory tasks improved. Side effects were reported more often on creatine than placebo. Vegetarians did not benefit more than meat eaters (Sandkühler et al., 2023).

The authors did not call it a dead end. Their read was that creatine might have a small beneficial effect, that larger studies are needed to confirm or rule it out, and that even a small effect could matter when scaled across many people over time. That is a fair and unglamorous conclusion, and it is a long way from what a supplement label will tell you.

One older trial adds a wrinkle. In 128 young women given 20 grams a day for five days, creatine did not change verbal fluency or vigilance overall, but vegetarians showed better memory than meat eaters, and everyone showed less variability in reaction times (Benton and Donohoe, 2010). The vegetarian angle makes mechanistic sense, since dietary creatine comes mostly from meat and fish, though the big 2023 trial did not reproduce it.

Where it looks most promising: when you are running on empty

The most striking recent finding is not about everyday brain function. It is about brain function when you are wrecked.

In a study of sleep deprivation, participants took a single high dose of creatine monohydrate, 0.35 grams per kilogram of body weight, during a 21-hour period of sleep restriction, with brain scans and cognitive tests at four timepoints through the night. Creatine shifted the brain's high-energy phosphate markers, helped prevent a drop in brain pH, and improved cognitive performance and processing speed compared with placebo (Gordji-Nejad et al., 2024).

Be careful with how you use that. It is one study. That is a very large single dose, far above a normal maintenance serving, and it was tested in an artificial all-nighter scenario, not on a Tuesday when you slept badly. It is a genuinely interesting proof of concept about creatine and brain energy under stress. It is not a dosing recommendation. If you are chronically underslept, creatine is not the fix. Your sleep is.

Aging: the part with the least hype and the most substance

Here is the irony. The best case for creatine past 40 is not the brain claim. It is the boring one.

A meta-analysis of 22 randomized controlled trials with 721 participants, mean ages 57 to 70, looked at creatine plus resistance training in older adults. Compared with training alone, adding creatine produced 1.37 kg more lean tissue mass, plus greater gains in chest press and leg press strength, across programs running 7 to 52 weeks (Chilibeck et al., 2017).

Keep it in proportion. A separate meta-analysis using direct imaging of muscle found only a very small added hypertrophy effect from creatine on top of training, roughly 0.10 to 0.16 cm of muscle thickness, and it actually trended better in younger adults than older ones (Burke et al., 2023).

So creatine is not a growth drug. It is a small, reliable edge stacked on top of work you still have to do. Past 40, when holding onto muscle and strength is one of the highest-value things you can do for how you age, a small reliable edge on a cheap supplement is a decent trade. Nobody is going to build an ad campaign around a gain of 1.37 kilograms over several months, which is exactly why you can trust it more than the ads.

The safety record

Creatine has a better safety file than almost anything else in the supplement aisle, mostly because it has been studied so heavily for so long.

The International Society of Sports Nutrition position stand concluded that short and long-term supplementation, at doses up to 30 grams a day for as long as five years, is safe and well tolerated in healthy people and across a range of patient populations from infants to the elderly (Kreider et al., 2017). That is a far wider safety margin than the 3 to 5 grams a day most men take.

A few honest notes. The kidney worry is the one that will not die, and it comes from a lab artifact: creatine supplementation can raise measured creatinine, which is a marker doctors use to estimate kidney function. That is a measurement effect, not evidence of damage in healthy people. Tell whoever orders your bloodwork that you take creatine so nobody misreads the number. If you have existing kidney disease, this is a conversation with your doctor, not a decision to make from a blog. Some men get bloating or stomach upset, which usually improves at lower daily doses without a loading phase. And in the largest cognition trial, side effects were reported more often on creatine than placebo, so "extremely well tolerated" is not the same as "zero effects."

How to take it if you decide to

The protocol is not complicated, which is part of the appeal.

Use creatine monohydrate. It is the form nearly every study used, and it is the cheapest. The fancier versions charge more to solve a problem monohydrate does not have. Take 3 to 5 grams a day, every day. Timing does not matter much, so take it whenever you will actually remember. You can skip the loading phase; it fills your stores faster but the endpoint is the same in a few weeks. Look for a third-party tested product, since supplements are loosely regulated.

Set your expectations at the right level. For training, expect a small but real edge. For memory and thinking, expect little to nothing if you are young and well rested, and possibly a modest benefit if you are older or under strain. If a bottle promises you sharper focus and a better mood, that promise is running well ahead of the evidence.

The verdict

Creatine outside the gym is a real research area, not a marketing invention, but the effects are smaller and less certain than the current wave of enthusiasm suggests. The memory benefit is small on average and concentrated in older adults. The biggest and best-run trial mostly came up empty in young healthy people. The sleep deprivation work is fascinating and very preliminary.

The strongest reason for a man over 40 to take creatine is still the original one, slightly reframed: it helps you hold onto muscle and strength while you train, it costs a few cents a day, and it has one of the cleanest safety records of anything you can buy. The brain stuff, if it pans out, is a bonus rather than the reason.

Common questions

Does creatine actually make you smarter?

Not in any dramatic way. A meta-analysis of randomized trials found a small overall improvement in memory, but the benefit was concentrated in older adults aged 66 to 76 and was essentially zero in younger people. The largest and best-controlled trial to date, 123 participants taking 5 grams a day for six weeks, found only a borderline effect on working memory and nothing on reasoning. The fair read is that creatine may have a small cognitive benefit, most likely in older men or men under real strain, and that bigger studies are still needed.

How much creatine should I take, and do I need to load?

Three to five grams of creatine monohydrate a day is the standard maintenance dose used in most research. You do not need a loading phase. Loading with roughly 20 grams a day for five to seven days fills your stores faster, but taking 3 to 5 grams daily gets you to the same place within a few weeks with less stomach upset. Timing barely matters, so take it whenever you will remember consistently.

Is creatine bad for your kidneys?

In healthy people, the evidence does not support that. The International Society of Sports Nutrition position stand concluded that supplementation up to 30 grams a day for as long as five years is safe and well tolerated, which is far above the 3 to 5 grams most men take. Without training, you are betting on the cognitive and aging research, which is real but modest and still unsettled. Given how cheap and well studied creatine is, it is a low-cost bet. Just do not expect it to do much on its own.
